Notification of School Absence

Please complete this form to notify school of your child's absence, It is essential you specify the reason for absence and not use the term 'unwell' as this information is recorded in our registers.

Please do not use this form for holiday leave.

Notification should be made on a daily basis, unless otherwise advised.

Please note all Schools follow a 48 hour exclusion period for vomitting and diarrhoea / sickness bugs.

Thank you for your submission the School Office will automatically be sent a copy of this form so they can update the register.
